Scholarship Application

Welcome to the 2019 ChiroTouch online Scholarship Application! Please submit all supporting documents through the scholarship application form below by October 15, 2019 to be considered for the $5,000 scholarship. If you are unable to submit your application online, please email scholarship@chirotouch.com with all supporting documents and include the Applicant Information in the body of the email.

 

General Instructions

Please submit the following along with a completed application form (incomplete applications will not be considered).

1. Copy of an approval letter from clinic administration as proof of clinic level and Good Clinic Standing;

2. One letter of recommendation, signed by the recommending party;

3. Response to Scholarship Essay Question: How will you use technology to achieve the best outcomes for your patients? (500-750 words)
